What health care would you want if you became too sick to tell the doctors yourself? Who would you like to speak for you? Do they know and understand your values regarding your health and end-of-life care? Are you certain you know your loved ones’ goals, values, and wishes regarding their care?

In honor of National Healthcare Decision Day, Cheshire Medical Center is offering opportunities throughout the month of April to learn about and participate in Advance Care Planning.
As part of Honoring Care Decisions, a Dartmouth-Hitchcock Health program designed to assist in Advance Care Planning for adults 18 or older, facilitators will be available to answer questions, work with individuals to establish advance directives, and facilitate conversations to help you or your family members express your wishes to each other and your designated Healthcare Agent so everyone is prepared if the need arises.
Join facilitators for thought-provoking conversations to help you reflect on your values, explore goals for treatment, form your wishes, and make them known. You may either take the first steps toward creating an advance directive, or complete one if you are ready.
Anyone who turns a complete advance directive or attends any of the following events will have the chance to win a gift basket from W.S. Badger!
